One of the following documents will be acceptable, and should be issued within the last 60 days of the request being made, referencing both your name and address:
- Bank statement
- Credit card statement
- Insurance Statement/Card
- Lease/Rental Agreement
- Mortgage Billing Statement
- Phone Bill
- Utility Bill
- Cable Bill
- Tax form
